Curative effect ofα-lipoicacid on automatic neuropathy in type 2 diabetes

Abstract:Objective To evaluate the efficiency of α-lipoic acid( ALA) on diabetic cardiac automatic neuropathy( CAN) .Methods Seventy-two patients were investigated by Ewing methodology and stepping stairs exercise.Then they were randomly divided into two groups: treatment group ( n=36),receiving ALA 600mg/d in normal saline 250 mL given by intravenous drip infusion; and control group(n=36),administered with vitamin C 500mg/d in normal saline 250 mL by intravenous drip infusion once a day;the course was 2 weeks.The efficiency was measured by the change of, before and after treatment, value of fasting blood glucose(FBG), 2 h postprandial blood glucose(PBG),total cholesterol ( TC ) , triglyceride ( TG ) , fructosamine ( FMN ) , systolic blood pressure ( SBP ) , malondialdehyde ( MDA ) , uncoupling protein -2 ( UCP -2 ) .The heart rate variability ( HRV ) was observed, compared before and after therapy, in terms of time domain such as the standard deviation of all N-N intervals( SDNN) , the standard deviation of the average of N-N intervals for each 5-min period over 24 h ( SDANN ) , the root mean square values of the standard deviation between adjacent normal number of intervals(RMSSD), percentage of N-N50ms in the total number of N-N intervals ( PNN50) and in the field of frequency domain such as high frequency( HF) , low frequency( LF) , LF/HF ratio.Results In the aspects of biochemical indexes, FBG, 2hPBG, TC and FMN in treatment group were improved, compared before and after therapy (P <0.05).The differences were not statistical different in control group;the MDA and UCP-2 were lower than before in treatment group, and MDA was decreased in control group as well(P<0.05).Heart rate variabilities were elevated after therapy in treatment group in time domain and frequency domain except for LF(P<0.05).HF and LF/HFratio were improved in control group ( P <0.05 ) .Conclusion Vagus nerve is destroyed earlier thansympathetic nerve. ALA is brought into full play in the improvementof the regeneration and restoration of demyelination, which would control a better balance between vagus and sympathetic nerve.
