Appilcation of CRUSADE scoring system in assessment of hemorrhage in patients with acute ST-segment elevation myocardial infarction treated with tirofiban
Abstract:Objective To evaluate the risk of bleeding in patients with acute ST -segment elevation myocardial infarction ( STEMI ) treated with tirofiban using CRUSADE scoring system. Methods CRUSADE scoring system was conducted in randomly collected 364 patients with STEMI in Hainan Province People's Hospital between January 2010 and June 2014.The patients were randomly divided into control group (160 cases) treated with dual anti-platelet agents (aspirin and clopidogrel) and tirofiban group (204 cases) .The incidence of major bleeding events were observed within 7 days in hospital.Results There were 30 major bleeding events in all 364 cases, and the incidence of bleeding was 8.2%.The highest hemorrhage incidence was gastrointestinal bleeding envents which accounted for 43%, followed by urinary tract hemorrhage.Incidence of intracranial, retroperitoneal and respiratory bleeding envents was relatively low.There were 9 cases of hemorrhage envents in control group during hospitalization, and the total incidence of bleeding was 5.6%.Hemorrhage incidence from extremely-low-risk group to extremely-high-risk group were 0%, 0%, 0.6%, 1.9%, 3.1%.There were 21 cases of hemorrhage envents in tirofiban group, and the total incidence of bleeding was 10.3%. Incidence of hemorrhage from extremely-low-risk group to extremely-high-risk group were 0%, 0%, 0.9%, 3.4%, 5.9%.The incidence of hemorrhage in high-risk group and extremely-high-risk group were significantly higher than those in other groups (P<0.05).Compared with the control group, the bleeding rates of high-risk group and extremely-high-risk group in tirofiban group were significantly increased (P<0.05).Conclusion The incidence rate of hemorrhage escalates with the increase of CRUSADE score.Compared with the control group, incidence of bleeding in high -risk group and extremely-high-risk group are significantly increased.CRUSADE scoring system is valuable in assessment of the risk of bleeding in patients with STEMI treated with tirofiban.
