A retrospective analysis of the risk factors for diuretic resistance in patients wti h type 1 cardiorenal syndrome
Abstract:Objective To explore the risk factors of diuretic resistance in patients with type 1 cardiorenal syndrome.Methods From January 2011 to July 2014, the patients with type 1 cardiorenal syndrome in Wenzhou People's Hospital were retrospectively analyzed.Depending on the occurrence of diuretic resistance, the patients were divided into the cardiorenal syndrome groups and the diuretic resistance groups, and the differences between the two groups were analyzed in many aspects, include gender, age, body mass index, mean arterial pressure, shock index, coronary heart disease, hypertension, diabetes, hyponatremia, anemia, central venous pressure, chronic heart failure, intravenous contrast agents, to accept continuous renal replacement therapy, etc.Finally, through multivariate Logistic regression analysis, the risk factors for the occurrence of diuretic resistance were analyzed.Results A total of 95 patients[aged from 17 ~96 (60.7 ±19.3) years, 70.5% male, 29.5%female] with type 1 cardiorenal syndrome were enrolled in this study.Among them, 58 patients were suffering from diuretic resistance.Logistic statistical results showed that:age, high central venous pressure and shock were independent risk factors for diuretic resistance, in which, shock was the most important independent predictor.While the intravenous contrast agents and mean arterial blood pressure were not risk factors. Conclusion The pathogen of diuretic resistance in patients with type 1 cardiorenal syndrome is complex, correcting shock and fluid control are effective methods of prevention.
