A clinical study of measurement of abdominal wall tension on patient with mechanical ventilation
Abstract:Objective To evaluate the intra-abdominal pressure and factors by measurement of abdominal wall tension.Methods Sixty patients with mechanical ventilation and indwelling urethral catheter admitted to ICU from October 2012 to Apirl 2014 were enrolled for a prospective study. According to the different positive end-expiratory pressure ( PEEP) the patients were divided into three groups:0~4 cm H2 O ( group A) , 5~9 cm H2 O ( group B) , 10~15 cm H2 O( group C) , every group has 20 patients.The abdominal wall tension ( AWT ) and urinary bladder pressure ( UBP ) were determined two hours after mechanical ventilation on the first day to the seventh day.Results A total of 60 patients were recruited in the study.A significantly linear correlation between AWT and UBP was observed (R=0.985, P<0.01), the regression equation was Y=-0.656 +8.231X (P<0.01). Compared with group A and B, AWT in group C was statistically significant (P<0.05).In the same group of different monitoring point time, comparison with 2 h, AWT on the first day to the seventh day were statistically significant ( P <0.05).Conclusion There was a significantly linear correlation between AWT and UBP. With PEEP level increased, the intra -abdominal pressure increased, especially in the early.Therefore, for the mechanical ventilation patients, early detection of intra -abdominal pressure may be beneficial for patients to choose suitable PEEP level.
