The clinical significance of endothelin -1 in sepsis
Abstract:Objective To explore the clinical significance of endothelin-1 (ET-1) in the diagnosis and treatment of sepsis. Methods The 82 patients were divided into three groups:noninfection group (8 cases), general infection group (17 cases) and sepsis group (57 cases).The APACHEⅡscore was calculated in each group 24 hours after admission.At the same time, the levels of ET-1 and PCT were detected.And then the difference of plasma ET -1 levels, PCT levels and APACHEⅡscore among the three groups were analyzed.Next, 57 patients in sepsis group was divided into three subgroups according to the APACHEⅡscore:﹤15 points for subgroupⅠ(27 cases), 15 to 24 points for subgroupⅡ(25 cases), 25 points or more for subgroupⅢ(5 cases).And then to compare the difference of plasma ET -1 levels and PCT levels among the three subgroups and do the correlation analysis between ET-1 level respectively with plasma PCT and APACHEⅡscore.On the basis of the level of ET -1 as 45 ng/L divided the patients into two groups, then calculate each mortality and compared the two groups with Chi -square test.Results The plasma levels of ET -1 and PCT in sepsis group were higher than noninfection group and general infection group (P<0.05).The same applies to the APACHEⅡscore.In sepsis group, the plasma levels of ET-1 and PCT were increased in turn with the deterioration of the illness (P<0.05).Pearson correlation analysis results show that the ET-1 and PCT levels and APACHEⅡscores were positively correlated (P<0.01).The death rate of the group of plasma ET-1 level >45 ng/L was greater than the group of <45 ng/L ( P <0.05 ) . Conclusion The plasma levels of ET-1 can be used as an indicator to assessment the severity and prognosis of sepsis.
