The tube cuff -leak test on elderly patients as a predictor for post -extubation stridor
Abstract:Objective To determine the value of the cuff -leak test ( CLT) for predicting post-extubation stridor ( PES) among elderly patients , and to assess the incidence and identify factors associated with the PES .Methods Fifty-six elderly patients intubated were analyzed .A CLT was conducted before each extubation .The cuff-leak volume ( CLV) and the cuff-leak ratio ( CLR) were calculated .Results The incidence of PES in CLT positive group was 9/14 , while in negative group was 6/42.There was no difference in age , gender, body weight, duration of mechanical ventilation and tracheal tube cuff pressure .There was significant difference in cuff -leak volume and cuff -leak ratio. Stridor was associated with body weight and period of intubation .Cuff-leak volume 232 mL and cuff-leak ratio 18.5%can be used to predict the occurrence of critical value of PES .These results provide a framework with which to identify patients at risk of developing a stridor after extubation .Conclusion The CLT failure was an predictor of PES , and a low cuff-leak volume should be used as an indication for either delaying extubation or initiating other specific therapy in elderly patients , especially older women, the body weight, or longer endotracheal intubation time .
