Effects of respiratory mechanics of different oxygen flow rate to drive inhalation of heparin in mechanically ventilated patients with acute exacerbation of chronic obstructive pulmonary disease
Abstract:Objective To investigate the effects of different oxygen flow rate driven inhalation of heparin on respiratory mechanics in mechanically ventilated patients with acute exacerbation of chronic obstructive pulmonary disease ( AECOPD ) .Methods Fifty-one patients with AECOPD mechanical ventilation were randomly divided into A , B and C three groups .Group A ( 17 cases ) were given atomizing inhalation with the oxygen flow rate of 5 L/min , group B ( 19 cases ) were given the oxygen flow rate of 7 L/min, group C ( 15 cases ) were given the oxygen flow rate of 9 L/min.Respiratory mechanics were detected before and 30 min, 1, 3 and 7 days after atomizing inhalation in all patients . Results Compared with group A and C , patients in group B had obviously improved peak pressure and platform pressure of airway, airway resistance, the total dynamic pulmonary compliance (P<0.05);but there was no statistical significance between group A and group C (P>0.05).Patients in group B had significantly improved peak pressure and platform pressure of airway , airway resistance , the total dynamic lung compliance at each time point compared with those before atomization ( P <0.05 );patients in group A had significantly changed indexed above mentioned at the time points after 30 min of atomization compared with those before atomization (P<0.05);but there were no significant changes in all time points of group C when compared with before atomization (P>0.05).Conclusion The best oxygen flow rate is 7 L/min for heparin inhalation in mechanically ventilated patients with AECOPD , it can reduce airway pressure and airway resistance and increase the total dynamic pulmonary compliance .
