Clinical value of prone position ventilation in treating pateints with acute respiratory distress syndrom e
Abstract:Objective To investigate the value of prone position ventilation in treating patients with acute respiratory distress syndrome ( ARDS ) .Methods Nine patients with moderate to severe ARDS were ventilated on prone position .Blood gas analysis had been done before and after prone position ventilation to obtain parameters such as PaO 2 , PaCO2 and PaO2/FiO2 .The vital signs and the dosage of vasoactive agent were recorded .Results Nine patients with moderate and severe ARDS were ventilated on prone position for 44 times.Three of the patients were died ( mortality 33 .3%) and six of them were survived (66.7%).Among the total 44 prone position ventilations, 37 times of them showed increased PaO 2/FiO 2(84.09%);31 times of them showed>10%elevation of PaO2/FiO2 (70.45%). The FiO2 reduced significantly after prone position ventilation , PaO2 and PaO2/FiO2 increased from (75.20 ±17.48)mm Hg and 159.15 ±38.64 to (85.64 ±15.97)mm Hg(P<0.05)and 199.83 ± 54.31(P<00.5), respectively.PaCO 2 decreased from (38.60 ±6.14) mm Hg to (33.88 ±3.98) mm Hg (P<0.05).The CVP ascended from (11.29 ±2.89)cm H 2 O to (12.66 ±2.64)cm H2O; heart rate raised from (90.29 ±13.50)bpm to (94.93 ±16.19)bpm(P<0.05); hemodynamics keep stable during prone position ventilation .Conclusion Prone position ventilation can significantly improve the oxygenation situation of patients with moderate to severe ARDS .It is convenient and safety , so it deserves further clinical application .
