Effect of orthopedic robot-assisted nail placement in treating calcaneal fractures and the impact on serum traumatic stress indicators

Abstract:Objective To analyze the effect of orthopedic robot-assisted nail placement in treating calcaneal fractures and the impact on serum traumatic stress indicators.Methods A total of 100 patients with calcaneal fractures who were admitted to the Department of Orthopedic Trauma at Taihe Hospital in Shiyan from May 2022 to May 2024 were selected as the study subjects.Using random number table method,they were divided into the observation group and the control group,with 50 cases in each group.The control group underwent surgery via a traditional lateral L-shaped incision,while the observation group underwent Kirschner wire prying/tarsal sinus incision+full-thread headless screw+robot-assisted surgery.The general situation of surgery,fracture reduction effect,and changes in serum traumatic stress indicators[C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]were recorded.Changes in imaging indices before surgery and at 12 months after surgery were compared between the two groups.Complications were recorded.Results The duration of surgery in the observation group(85.79±8.65)min was longer than that in the control group(65.41±6.72)min,and the frequency of intraoperative fluoroscopy was lower than that in the control group[(36.12±3.74)times vs.(38.18±4.37)times].Fracture healing time and hospital stay[(3.02±0.33)months and(5.62±0.63)days]were shorter than those in the control group[(3.37±0.36)months and(8.79±0.94)days](P<0.05).Intraoperative blood loss and visual analogue scale(VAS)scores on postoperative days 1-3 in the observation group[(21.05±2.23)ml and(2.85±0.31)]were lower than those in the control group[(46.11±4.72)ml and(3.47±0.38)](P<0.05).At 12 months postoperatively,the American Orthopaedic Foot&Ankle Society(AOFAS)ankle-hindfoot score in the observation group(83.44±8.72)was higher than that in the control group(75.18±7.63)(P<0.05),and the excellent-good rate of fracture reduction in the observation group(96%)was higher than that in the control group(84%)(P<0.05).At 1 week postoperatively,serum CRP,IL-6,and TNF-α levels in the observation group[(9.43±0.96)mg/L,(35.18±3.62)ng/L,(52.15±5.34)pg/ml]were lower than those in the control group[(10.17±1.45)mg/L,(40.75±4.27)ng/L,(54.73±5.67)pg/ml](P<0.05).At 12 months postoperatively,calcaneal length,height,Böhler's angle,and Gissane's angle in the observation group[(77.45±7.89)mm,(40.25±4.18)mm,(26.38±2.72)°,(131.26±13.51)°]were larger than those in the control group[(73.26±7.61)mm,(37.77±3.79)mm,(24.89±2.57)°,(123.88±13.37)°](P<0.05).There was no statistically significant difference in the risk of complications between the two groups(P>0.05).Conclusions Robot-assisted nail placement is effective in treating calcaneal fractures,which can reduce pain,improve the accuracy of localization,promote fracture healing,reduce traumatic stress response,and improve imaging indicators,with good safety.It is worthy of promotion and application in clinical practice.
