Correlation between sarcopenia,bone mineral density,and serum 25-hydroxyvitamin D3 levels in elderly patients with hip fractures

Abstract:Objective To investigate the correlation between sarcopenia,bone mineral density(BMD),and serum 25-hydroxyvitamin D3[25(OH)D3]levels in elderly patients with hip fractures.Methods A total of 200 patients with hip fractures(100 femoral neck fractures and 100 intertrochanteric fractures)who were treated in the Department of Orthopedics at our hospital from October 2020 to October 2024 were enrolled as the observation group.All examinations were performed within 48 hours after fracture occurrence and prior to surgery.An age-matched control group of 100 community-dwelling elderly individuals without fractures was randomly selected from the health examination center.Hemoglobin,albumin,parathyroid hormone(PTH),osteocalcin(OC),procollagen type 1 N-terminal propeptide(P1NP),C-terminal telopeptide of type 1 collagen(CTX),25(OH)D3,serum calcium(Ca),appendicular skeletal muscle index(ASMI),BMD,handgrip strength,and 6-meter walking speed were compared between hip fracture patients and controls,as well as between sarcopenic and non-sarcopenic patients within the fracture cohort.Results Compared with the control group,the observation group showed significantly lower handgrip strength,serum 25(OH)D3 levels,and BMD at the lumbar spine(L1-4),femoral neck,and total hip(all P<0.01).Within the observation group,patients were classified into sarcopenia and non-sarcopenia subgroups.Compared with the non-sarcopenia group,the sarcopenia group had significantly lower body mass index(BMI),handgrip strength,ASMI,6-meter walking speed,hemoglobin,albumin,and BMD at L1-4,femoral neck,and total hip,but higher age(all P<0.05).logistic regression analysis identified low BMI,reduced serum 25(OH)D3,and decreased BMD at L1-4,femoral neck,and total hip as independent risk factors for sarcopenia(all P<0.05).Conclusions Sarcopenia is highly prevalent among elderly patients with osteoporotic hip fractures.Low BMI,reduced serum 25(OH)D3 levels,and decreased BMD at L1-4,femoral neck,and total hip are significant risk factors.Nutritional optimization and vitamin D supplementation may help reduce the risk of sarcopenia in this population.
