Research on the application of anchor combined with Broström-Gould technique in the treatment of anterior talofibular ligament injury

Abstract:Objective To investigate the clinical efficacy of suture anchor combined with Broström-Gould technique in the treatment of anterior talofibular ligament(ATFL)injuries.Methods A single-center retrospective analysis was conducted on 48 patients(48 ankles)with persistent lateral ankle instability due to ATFL injury after conservative treatment.All were admitted to the Department of Orthopedics,The hospital of 82nd group army PLA,from March 2020 to December 2023.All patients underwent surgical repair using suture anchor combined with Broström-Gould technique.Perioperative indicators,including operative time,intraoperative blood loss,average length of stay(ALOS),and intraoperative improvements in anterior talar translation(ATT)and talar tilt degree(TT°)were analyzed.Postoperative magnetic resonance imaging(MRI)was utilized to assess ATFL healing.Pain and functional outcomes were evaluated using the Visual Analogue Scale(VAS)and American Orthopaedic Foot&Ankle Society Score(AOFAS)preoperatively and postoperatively,with ankle range of motion(AROM)measured at final follow-up.Results All 48 patients treated with the anchor-based Broström-Gould technique achieved excellent outcomes.Following a follow-up period of 12 to 34(20.5±6.0)months,the excellent-good rate was 100%.This surgical approach optimized operative procedures and minimized trauma,with an average operation time of 30 to 50(39.58±8.49)minutes,an incision length of(3.50±0.60)cm,and intraoperative blood loss of 5 to 15(10.16±3.12)ml.Statistically significant differences were observed in ATT and talar tilt(TT)between preoperation and the final follow-up:[(10.60±1.16)mm vs.(3.66±1.09)mm]for ATT,and[(13.06±4.33)° vs.(4.33±1.44)°]for TT(all P<0.001).The VAS score improved significantly from(3.91±0.84)preoperatively to(1.12±0.86)postoperatively.The AOFAS score,indicating moderate-to-severe functional limitation preoperatively(71.25±4.28),significantly increased to(92.79±2.07)postoperatively(P<0.001).At the final follow-up,both dorsiflexion and plantarflexion angles significantly improved compared with preoperative values:[(25.10±2.40)° vs.(18.20±3.50)°;(39.80±3.20)° vs.(32.40±4.10)°],respectively(P<0.05),approaching the functional level of a normal ankle joint.Conclusions The suture anchor combined with Broström-Gould technique offers advantages including shorter operative time,procedural simplicity,excellent postoperative stability,rapid recovery,and low complication rates,making it a clinically valuable approach for ATFL injury repair.
