The staged treatment of Badia arthroscopy for the first carpal metacarpal arthritis

Abstract:Objective To investigate the role of arthroscopy in the endoscopic staging and adjuvant treatment of the first carpal metacarpal arthritis.Methods From August 2018 to July 2022,14 patients(15 joints)with the first carpal metacarpal arthritis were treated with arthroscopy,including 6 males(1 bilateral)and 8 females(unilateral).Age:(52.1±6.9)years(range:31-67 years).Arthroscopy diagnosis stage and surgical method:Badia arthroscopic stage Ⅰ(2 cases)underwent synovial debridement and ligamentous shrinkage;stage Ⅱ(4 cases)underwent synovial debridement,ligamentous shrinkage and K-wire internal fixation;2 cases underwent free body extraction;9 joints of stage Ⅲ osteoarthritis(8 cases)underwent arthroscopic-assisted fusion.1 joint underwent arthroscopic semi-resection of most trapezium with thread button plate suspension.The length and healing of the incision were observed after surgery.The postoperative effect was evaluated by comparison of visual analog scale(VAS),upper limb dysfunction score(DASH)and thumb and finger side pinch strength before and after surgery.Results Arthroscopic evaluation and staging of the first carpal metacarpal joint lesions were successfully performed in all patients.All 14 patients with 15 joints were followed up twice after the operation,for 3 to 6 months and 6 to 13 months,respectively.All incisions healed at stage Ⅰ without infection.The mean VAS score decreased from 6.8 to 1.2 at 2 follow-up visits.After removal of the free body,the patients felt no pain when moving the joint.The mean DASH score decreased from 56.3 to 15.5,and the percentage of the affected side to the healthy side increased from 75.4%(55.0%-98.0%)preoperatively to 93.6%(67.0%-120.0%)postoperatively.Conclusions Arthroscopic technique has the advantages of small incision,less trauma and less complications in the treatment of first carpal metacarpal arthritis.It can provide accurate assessment and staging of intraarticular lesions,and appropriate and effective treatment for the corresponding stage.
