Clinical study of intraoperative accidental hypothermia on postoperative recovery in elderly patients with intertrochanteric femoral fractures

Abstract:Objective To analyze the effect of accidental intraoperative hypothermia on postoperative recovery of elderly patients with intertrochanteric femoral fracture(IFF).Methods 344 elderly IFF patients who were surgically treated between February 2022 and April 2024 in our hospital were selected.A total of 72 patients with intraoperative accidental hypothermia and 72 patients with normal intraoperative body temperature were finally included after 1:1 propensity score matching.Perioperative indicators(operation time,intraoperative blood loss,intraoperative blood transfusion,hospitalization time,postoperative complications)were compared between the two groups of patients.The coagulation function indexes and inflammatory response indexes of the two groups of patients were compared.Harris hip score(HHS)was used to assess the hip function of the two groups of patients.The Barthel index(BI)rating scale was used to assess the ability to perform activities of daily living in both groups of patients.Results Surgical time,intraoperative blood loss,intraoperative blood transfusion and hospitalization time were higher in the intraoperative accidental hypothermia group than in the intraoperative normal body temperature group(P<0.05).The total postoperative complication rate was higher in the accidental intraoperative hypothermia than in the intraoperative normothermia group(20.8%vs.6.9%,P=0.016).At 1,2 and 3 days postoperatively,thrombin time(TT),plasma activated partial thromboplastin time(APTT),prothrombin time(PT)time and leukocyte Interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor(TNF-α)levels were significantly higher than those in the intraoperative normothermia group(P<0.05).At 3 months postoperatively,the HHS and BI scores in the intraoperative accidental hypothermia group were lower than those in the intraoperative normal body temperature group(P<0.05).Conclusions Unexpected intraoperative hypothermia can lead to coagulation disorders in elderly patients with IFF,exacerbate inflammatory responses,increase postoperative complications,and impair the patient's recovery of hip function.
