Biportal endoscopic revision surgery for postoperative cage migration after lumbar interbody fusion:case series and technical note

Abstract:Objective To evaluate the efficacy of biportal endoscopic revision surgery(BERS)in the treatment of postoperative cage migration(PCM)following lumbar interbody fusion and to investigate the risk factors,impact,and surgical strategies for PCM.Methods A retrospective study was conducted on six patients who developed PCM after endoscopic fusion between January 2019 and January 2022.All patients underwent BERS.The surgical procedures were meticulously documented,and the visual analogue scale(VAS)scores for back and leg pain,as well as the Oswestry Disability Index(ODI),were recorded preoperatively and at the one-year postoperative follow-up.Results Significant reductions in VAS and ODI scores were observed at the one-year follow-up.The mean preoperative VAS scores for back pain and leg pain were(7.00±0.89)and(6.83±0.75),respectively,while the mean ODI score was(45.00±4.34).At one year postoperatively,the mean VAS scores for back pain and leg pain had decreased to(2.00±0.63)and(2.17±0.75),respectively,with the mean ODI score improving to(20.00±4.73).Conclusions PCM is a rare complication of lumbar interbody fusion.Endplate injury,an undersized cage,and posterior cage placement are significant risk factors for its occurrence.BERS is a viable surgical revision approach for PCM and may serve as a valuable treatment option.
