Tuberculous synovitis of the hip with rice body:a case report and literature review

Abstract：Objective To report one case of tuberculous synovitis of the hip joint with rice body formation,and a total of 86 cases were reviewed in the literature to discuss the clinical characteristics of joint rice body.Methods The diagnosis and treatment process of a patient with hip tuberculous synovitis and rice body treated in our hospital were introduced and analyzed.Retrieve the Chinese and English databases,and items included pathological mechanism,location,primary disease,differential diagnosis and treatment of rice body.Results The pathological mechanisms and components of rice body caused by different disease were different.It was easily misdiagnosed as synovial chondroma and pigmented villonodular synovitis.MRI was the most commonly used imaging examination.There were 46 cases in the shoulder joint,14 cases in the wrist joint,8 cases in the knee joint,10 cases in the metacarpophalangeal joint,5 cases in the hip joint,and 3 cases in the ankle joint.Regarding primary diseases,non-specific in 48 cases,rheumatoid in 18 cases,tuberculosis in 18 cases and non-tuberculous mycobacterium in 2 cases.The treatment methods were mainly divided into open surgery and arthroscopic surgery.Conclusions Rice body is a special form of synovial inflammation.The diagnosis can be confirmed by combining clinical manifestations,imaging results,molecular biology results and pathological results.Surgical removal can achieve satisfactory clinical results.
