Analysis of risk factors and short-term prognosis of elderly patients with hip fracture admitted to intensive care unit after operation

Abstract:Objective To investigate the incidence and independent risk factors of hip fracture in elderly patients admitted to intensive care unit(ICU)postoperatively and the effects on short-term prognosis.Methods The clinical data of 1444 elderly patients with hip fracture admitted to the Department of Orthopedics of Shenzhen Second People's Hospital from January 2012 to December 2021 were retrospectively analyzed.Patients were divided into ICU group and non-ICU group according to whether they were transferred to ICU within 24 hours after surgery.Univariate and multivariate Logistic regression were used to analyze the independent risk factors of ICU admission.The postoperative hospitalization days,incidence of pulmonary infection and in-hospital mortality were compared between ICU group and non-ICU group.Results The incidence of postoperative ICU admission in elderly patients with hip fracture was 2.15%(31/1444).Univariate analysis showed that there were statistically significant differences in age,comorbidities,coronary heart disease,stroke,Parkinson's disease,number of comorbidities,albumin,creatinine,urea nitrogen and ASA grades between the two groups(P<0.05).Multivariate Logistic regression analysis showed coronary heart disease(OR:2.450;95%CI:1.104-5.438),stroke(OR:3.068;95%CI:1.430-6.582),Parkinson's disease(OR:7.422;95%CI:2.254-24.432),albumin(OR:0.908;95%CI:0.847-0.974),creatinine(OR:1.003;95%CI:1.000-1.005),ASA classification≥Ⅲ(OR:5.998;95%CI:1.388-25.924)were independent risk factors for postoperative ICU admission in elderly patients with hip fracture(P<0.05).Patients in ICU had longer hospital stay,higher incidence of pulmonary infection and higher in-hospital mortality(P<0.05).Conclusions Elderly patients with hip fracture are at higher risk of being admitted to ICU after operation.Coronary heart disease,stroke,Parkinson's disease,albumin,creatinine,and ASA grades are independent risk factors for ICU admission.Patients admitted to the ICU after surgery have longer hospital stays,higher rates of lung infections,and higher in-hospital mortality.
