Short-term curative effect of FNS internal fixation on unstable femoral neck fracture

Abstract:Objective To evaluate the short-term efficacy of cannulated screws and femoral neck system(FNS)in the unstable femoral neck fractures and compare their clinical results.Methods A retrospectively analysis of 69 unstable femoral neck fracture patients admitted to our hospital from January 2022 to October 2023 was conducted,including 25 patients in cannulated screw fixation group and 44 cases in FNS internal fixation group.The operation time,intraoperative blood loss,intraoperative C-arm fluoroscopy times,postoperative visual analogue scale(VAS),Harris hip function score,postoperative X-ray fracture healing time and related complications were recorded.Results No statistically significant difference was found in sex,age,operation time,postoperative VAS score(1 month,3 months)and Harris score(1 month,3 months,6 months and 12 months)between the two groups(P>0.05).The CCS group demonstrated advantages in intraoperative blood loss[(29.24±10.50)ml vs.(51.86±27.02)ml,P<0.05]and VAS at 2 days[2.00(2.00,3.00)vs.3.00(2.00,4.00),P<0.05],but required significantly more fluoroscopy[24.00(22.00,27.50)times vs.16.00(10.25,19.75)times,P<0.05].During follow-up,the FNS group exhibited superior anatomical restoration,with reduced femoral neck shortening[1.70(1.30,2.30)mm vs.4.70(2.65,6.75)mm,P<0.05]and shorter fracture healing time[3.00 months(2.00,3.00)vs.4.00 months(3.00,5.00),P<0.05].Complication rates showed no statistical difference.Conclusions FNS achieves comparable minimally invasive benefits over CCS,while significantly reducing fluoroscopy exposure,shortening fracture healing time,and minimizing femoral neck shortening.Despite slightly inferior early pain management,FNS demonstrates superior anatomical stability and functional recovery,establishing it as a preferable option for unstable femoral neck fractures.
