Effect of ERAS programme adherence on functional recovery of the ankle in patients undergoing ankle fracture surgery

Abstract:Objective To analyze the relationship between Enhanced Recovery After Surgery(ERAS)programme and ankle functional recovery in patients undergoing ankle fracture surgery.Methods A total of 137 patients admitted to our hospital between July 2019 and June 2022 for ankle fracture surgery were analyzed.Perioperative indicators(admission to surgery,fracture onset to surgery,length of surgery,length of hospital stay,and complications),pain levels,and ankle function were compared between patients with low and high adherence.Multiple linear regression was used to analyse the relationship between ERAS compliance and ankle function at 3 and 6 months postoperatively.Results The overall adherence rate to the ERAS protocol in 137 patients undergoing ankle fracture surgery was 72.96%,with low adherence to early education,low adherence to temperature management and early activity,and high adherence to early fasting and multimodal analgesia(preoperative intraoperative and postoperative analgesia).Patients were categorized into a low adherence group(<73.08%,n=59)and a high adherence group(≥73.08%,n=78),based on a median ERAS protocol of 73.08%as a reference value.The age of patients with low adherence group was lower than that of patients with high adherence group(P<0.05);the length of hospital stay and the proportion of perioperative complications were higher in patients with low adherence group than in patients with high adherence group(P<0.05);the resting and active VAS were higher in patients with low adherence group than in patients with high adherence group at 3 and 6 months postoperatively(P<0.05);and the AOFAS scores were lower in patients with low adherence group than in patients with high adherence group at 3 and 6 months postoperatively(P<0.05);multiple linear regression analyses found that linear regression analyses of AOFAS scores in ankle fracture patients at both 3 and 6 months postoperatively confirmed that ERAS adherence was closely related to AOFAS scores(P<0.05).Conclusions Short-term rehabilitation is better in patients undergoing ankle fracture surgery with high adherence to the ERAS protocol.
