Analysis of the effects of progressive dynamic balance training on gait balance,function and quality of life in total knee arthroplasty patients

Abstract:Objective To explore the effects of progressive dynamic balance training on gait balance,function and quality of life of total knee arthroplasty(TKA)patients.Methods 96 TKA patients who were surgically treated between March 2021 and March 2023 in our hospital were selected.The patients were divided into a control group and an observation group according to the random number table method,with 48 cases in each group.Conventional balance training was used in the control group,and progressive dynamic balance training was used in the observation group.The patients'gait(kinematic,spatio-temporal and kinetic parameters)was analyzed by using the Vicon 3D gait capture system and the embedded force platform for the 10-meter walkway test;the patients'static balance was tested by the FDM-L multifunctional force plate,and the dynamic balance was tested by the Timed Up and Go test(TUG);the patients'dynamic balance was tested by the Western Ontario and McMaster Western Ontario and McMaster Universities osteoarthritis index(WOMAC);the knee injury and osteoarthritis outcome score(KOOS)was used to assess the patients'knee joint ability;the knee outcome survey-activities of daily living scale(KOS-ADLS)was used to assess the patients'quality of life.Results At 3 months after surgery,the minimum flexion angle and maximum flexion angle in standing,maximum flexion angle in swinging,step speed,step length,step frequency,knee rotation,inversion and flexion of patients in the observation group were higher than those of the control group(P<0.05);the 95%CI ellipse area(CEA),path length path length(PL)and average velocity(AV)and TUG were higher than those of the control group(P<0.05);at 1 and 3 months after surgery,the WOMAC of patients in the observation group was lower than that of the control group(P<0.05),the KOOS was higher than that of the control group(P<0.05),and the KOS-ADLS of patients in the observation group was higher than those of the control group(P<0.05).Conclusions Progressive dynamic balance training can effectively improve gait balance,function and quality of life of TKA patients and is suitable for clinical promotion.
