Comparative study on the clinical efficacy of retrograde femoral intramedullary nail plus lateral locking plate and medial-lateral double plate in the treatment of AO/OTA 33C distal femoral fracture

Abstract：Objective To compare the clinical efficacy of retrograde femoral intramedullary nail with lateral locking plate and medial and lateral femoral double plate in the treatment of AO/OTA 33C distal femoral fracture.Methods From June 2017 to December 2022,78 cases of AO/OTA 33C distal femoral fractures treated in our hospital were analyzed retrospectively,including 42 cases of femoral retrograde intramedullary nail plus lateral locking plate(nail plate group)and 36 cases of femoral medial and lateral double plate(double plate group).The operation time,intraoperative blood loss,weight-bearing time,fracture healing time,visual analogue scale(VAS),range of motion(ROM),hospital for special surgery(HSS)and complications of the two groups were recorded.Results There was no significant difference in operation time,intraoperative blood loss,weight-bearing time and fracture healing time between nail plate group and double plate group(P>0.05).There was no significant difference in VAS score,knee joint range of motion and HSS score between nail plate group and double plate group at 3 and 6 months after operation(P>0.05).There were 5 cases of postoperative complications in nail plate group:2 cases of knee joint stiffness,1 case of limb shortening and 2 cases of wound infection.There were 6 cases of postoperative complications in the double plate group:1 case of knee joint stiffness,2 cases of delayed healing and 3 cases of wound infection.There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions Combined fixation has become a trend in the treatment of AO/OTA 33C distal femoral fractures.Retrograde femoral intramedullary nail with lateral locking plate and medial-lateral double-plate fixation are both effective schemes for the treatment,which have the advantages of reliable fixation,rapid recovery and satisfactory effects.
