Abstract:Giant cell tumor of bone (GCTB) is an aggressive primary bone tumor with a tendency for local recurrence, accounting for 10% to 20% of all primary bone tumors. Surgery is the preferred treatment in clinical practice. For refractory GCTB that is unresectable, difficult to resect, recurrent after surgery, or has metastasized, non-surgical treatments such as RANKL inhibitors, arterial embolization, and radiotherapy can be considered [1]. Among these, RANKL inhibitors, represented by denosumab, have shown good clinical efficacy and biological safety in the treatment of such patients [2-6], and some scholars believe that the treatment of refractory GCTB has entered the era of denosumab [7]. However, with the widespread use of this drug, many clinical issues remain, such as indications for clinical application, monitoring during medication, prevention and management of complications, etc. Currently, there are no relevant expert consensuses or guidelines to guide clinical practice and standardize clinical behavior both domestically and internationally. To address this, the Chinese Society of Geriatrics and Geriatric Medicine, Orthopedic Branch, and the Shandong Cancer Association, Bone and Soft Tissue Tumor Branch, organized domestic experts to select 14 clinical questions most concerning orthopedic surgeons. Through steps such as evidence retrieval, evaluation of evidence quality, and establishment of recommendation directions and strengths, 14 recommendations were finally formed to answer related issues regarding the use of RANKL inhibitors in the treatment of GCTB. The aim is to provide clinical physicians with referenceable guidance for clinical application.
