Finite element analysis of the effect of different reconstruction methods on the stability of type ⅠB bone defects after resection of pelvic region Ⅰ-Ⅳ tumors

Abstract:Objective To compare the effects of four reconstruction methods on the stability of pelvic tumors involving sacral iliac joints by finite element analysis,so as to select the best reconstruction method.Methods The CT data of the required area of an adult male was collected,and the normal finite element model was obtained by extracting,smoothing,reconstructing the 3D model,and meshing.A model after resection was obtained using the cutting function in Solidworks.And it was assembled together with the nail rod system established in this software.Different reconstruction methods were simulated by different settings.A finite element model of 4 different reconstruction methods was established.Group A:pelvic ring fusion group;Group B:pelvic ring + contralateral sacroiliac joint fusion group;Group C:pelvic ring + lumbar 5-sacral 1 fusion group;Group D:pelvic ring + lumbar 5 sacral 1 + contralateral sacroiliac joint fusion group.A downward 600 N vertical load on the L5 upper surface of the four reconstructed models was applied,and the stress and displacement of the lower pelvis and internal fixation under different fixation methods were compared.Results Outcomes under stress:Group A,maximum von-Mises(Mises)stress 633.12 MPa,maximum displacement 19.962 mm;Group B,maximum Mises stress 603.69 MPa,maximum displacement 18.835 mm;Group C:maximum Misses stress 556.28 MPa,maximum displacement 78.610 mm;Group D:maximum Misses stress 610.14 MPa,maximum displacement 79.041 mm.The results showed that the maximum displacement of Group B was the smallest among 4 groups.The maximum Misses stress in Group C was minimal,but its maximum displacement was much greater than that of Group B.Conclusions Combining the Misses stress and displacement,Group B is superior in mechanical property.The reconstruction scheme of the pelvic ring + contralateral sacroiliac joint fusion is excellent.
