Effects of ultrasound guided fascia iliaca compartment nerve block on postoperative analgesia and functional exercise in elderly patients after knee arthroplasty

Abstract:Objective To evaluate effects of ultrasound guided fascia iliaca compartment nerve block on postoperative analgesia and functional exercise in elderly patients after knee arthroplasty. Methods All 68 cases were randomly divided into the control group (n =34) and observation group (n =34). All patients completed the operation under general anesthesia. Patients in the observation group underwent ultrasound guided fascia iliaca compartment nerve block, while patients in the control group received routine treatment. The VAS pain score in resting and active state, incidence of adverse reactions, satisfaction rate, functional exercises were recorded 12 h, 24 h, 48 h, 1 week after operation. Results The VAS scores at rest in the control group [12 h (3.9 ± 0.6), 24 h (3.9 ± 0.4), 48 h (3.8 ± 0.4)] were significantly higher than that in the observation group [12 h (3.5 ± 0.3), 24 h (3.6 ± 0.3), 48 h (3.6 ± 0.4)] (P < 0.05). The VAS scores in the active state in the control group [12 h (6.2 ± 1.0), 24 h (6.0 ±0.9), 48 h (6.1 ± 1.1)] were significantly higher than that in the observation group [12 h (5.4 ± 1.2), 24 h (5.5 ± 1.2), 48 h (5.6 ± 1.2)] (P < 0.05). In the control group, 17 patients developed adverse reactions (nausea and vomiting in 7, drowsiness in 2, pruritus in 2 and numbness in 6). In the observation group, 9 patients developed adverse reactions (nausea and vomiting in 3, drowsiness in 1, pruritus in 3 and numbness in 6) (P < 0.05). The number of people who participated in functional training in the control group (0 within 12 hours, 20.6% at 24 hours, 50.0% at 48 hours, 76.5% at 1 week after operation) was less than that in the observation group (12.5% within 12 hours, 47.1% at 24 hours, 73.5% at 48 hours, 94.1% at 1 week after operation) (P < 0.05). Conclusions Ultrasound guided fascia iliaca compartment nerve block has significant effects on the postoperative analgesia and functional exercises in elderly patients after knee arthroplasty, which also reduces adverse reactions and improves patients' satisfaction with the analgesic effects.
