Effects of tranexamic acid intra-articular injection on drainage, hemoglobin and coagulation function after total knee arthroplasty

Abstract:Objective To investigate effects of tranexamic acid intra-articular injection on drainage, hemoglobin (Hb) and coagulation functions after total knee arthroplasty (TKA). Methods All 75 TKA patients were divided into the control group (no tranexamic acid intra-articular injection) and observation group (tranexamic acid intra-articular injection). The blood transfusion volume, drainage 24 h postoperatively, hidden blood loss, total blood loss, index of blood clotting, Hb, CRP and D-dimer levels were analyzed. Results The volume of blood transfusion was (352.11 ± 45.21) ml in the observation group and (478.98 ± 51.22) ml in the control group (P < 0.05); 24 hours after operation, the drainage volume was (241.22 ± 158.41) ml in the observation group and (452.85 ± 185.22) ml in the control group (P < 0.05); The hidden blood loss was (407.55 ± 85.66) ml in the observation group and (584.11 ±100.11) ml in the control group (P < 0.05); The total blood loss was (821.33 ± 101.15) ml in the observation group and (955.44 ± 154.21) ml in the control group (P < 0.05); The preoperative Hb was (130.89 ± 11.58) g / L in the observation group and (133.14 ± 10.14) g / L in the control group (P < 0.05); On the 1st day after operation, the Hb was (111.11 ± 8.11) g / L in the observation group and (96.58 ± 7.14) g / L in the control group (P < 0.05); On the 3rd day after operation, the Hb was (102.64 ± 7.47) g / L in the observation group and (87.69 ± 6.11) g / L in the control group (P < 0.05); On the 7th day after operation, the Hb was (100.74 ± 6.91) g / L in the observation group and (82.88 ± 6.10) g / L in the control group (P < 0.05). The postoperative levels of CRP, D-dimer increased. HSS results were improved. There were no significant differences in complication rate, fibrinogen, prothrombin time and activated partial thromboplastin time between the 2 groups (P > 0.05). Postoperative CRP in the observation group was lower than that in the control group (P < 0.05), but there were no significant differences in the level of D-dimer between the 2 groups (P > 0.05). Conclusions Tranexamic acid intra-articular injection after TKA can reduce postoperative drainage volume, total blood loss, and improve knee functions, without increasing the risk of deep vein thrombosis (DVT).
