A mortality and complication analysis of cemented and uncemented hemiarthroplasty for displaced femoral neck fractures in elderly: a Meta analysis

Abstract:Objective To compare cemented and uncemented hemiarthroplasty for displaced femoral neck fractures in elderly patients in terms of mortality and complications. Methods Pubmed, EMBASE, Cochrane library, CNKI, Wanfang and VIP database were searched till Feburary 2017. Randomized controlled trials ( RCTs ) comparing cemented and uncemented hemiarthroplasty in terms of mortality et al. were included. Intraoperative blood loss, orthopedic or non-orthopedic related complications, and mortality ( within half or one year ) were compared. The meta analysis was performed with RevMan 5.3. Results Seven eligible RCTs involving 1210 patients were included in this review. The rate of orthopedic-related complications in the uncemented group was higher than those in the cemented group with statistical significance ( P < 0.05 ). No statistical significance existed in the intraoperative blood loss, non-orthopedic related complications, and mortality ( within half or one year ) ( P > 0.05 ). Conclusions Cemented hemiarthroplasty has advantages of less orthopedic-related complications ( periprosthetic fractures, dislocation, loosening ), reducing the occurrence of the second surgery, improving the surgery safety effectively, and will not increase the postoperative non-orthopedic related complications and mortality. It should be recommended as an optimal choice.
