Diagnosis and treatment of lateral clavicular fracture in children

Abstract:Objective To evaluate the clinical efficacy of different methods for the treatment of lateral clavicular fractures in children. Methods From Jan 2008 to Jan 2016, 31 children ( male 22, female 9, left 19, right 12 ) of lateral clavicular fracture were evaluated, with an average age of ( 8 ± 2.3 ) years ( range: 3 - 16 years ). Rockwood: I 7, II 12, III 7, IV 4, V 1 with acromioclavicular dislocation. Eight cases ( age 7 to 16 years, average 10 ± 2.7 years ) received open reduction with Kirschner wire tension. Twenty-three cases ( age 4 to 16 years, average 7 ± 1.9 years ) received conservative treatment. Joint functions were evaluated by Constant-Murley Scores. Results A total of 26 cases were followed up ( ranging from 6 months to 7 years, with an average of 56 months ). Evaluated by Constant-Murley Scores, all cases had excellent or good joint functions. Two cases receiving conservative treatment had protuberant deformity on the injury shoulder. All cases in the operation group had no malformations. Conclusions Both operation and conservative treatment of the lateral clavicular fracture can achiere good shoulder functions. To elder children, especially to those with fractures of Rockwood III, IV and V, open reduction should be used to avoid the malformation and duplication of the clavicle.
