Long-term observation of curative effect of administrating sodium ibandronate after cyst curettage and bone grafting in the treatment of spinal giant cell tumor

Abstract:Objective To investigate the long-term curative effect of administrating sodium ibandronate after cyst curettage and bone grafting in the treatment of spinal giant cell tumor.Methods From July 2005 to January 2010, a total of 62 patients with spinal giant cell tumor conifrmed by preoperative needle biopsy were treated in 6 hospitals in North China. Among them, there were 22 males and 40 females, whose mean age was 28.2 years ( range: 19 - 40 years ). There were 39 patients with giant cell tumor in Campanacci stage II and 23 patients in Campanacci stage III. There were 22 cases of giant cell tumor at the thoracic vertebra, 10 cases at the lumbar vertebra and 30 cases at the sacral vertebra. All the patients underwent spinal giant cell tumor resection and bone grafting. In the ibandronate group, 32 patients were administrated with 4mg sodium ibandronate each month for 1 years. In the control group, 30 patients were not given any bisphosphonates. All the patients underwent X-ray, CT and MRI examinations. The operation situation, clinical effect, ossification rate of lesions and recurrence rate were compared between the 2 groups.Results The patients were followed up for 60 - 78 months ( mean 67 months ). The differences in time of operation, amount of bleeding and wound healing were not statistically signiifcant between the 2 groups (P > 0.05 ). The improvement of visual analogue scale ( VAS ) was ( 5.4 ± 1.6 ) points in the ibandronate group, which was ( 3.7 ± 2.6 ) points in the control group. The improvement of Oswestry disability index ( ODI ) was ( 45.4 ± 16.6 ) pointsin the ibandronate group, which was ( 32.9 ± 13.6 ) points in the control group. The differences in the VAS and ODI scores were statistically signiifcant between the 2 groups (P< 0.05 ). The recurrence rate was 6.3% in the ibandronate group, which was 46.7% in the control group, and the differences between the 2 groups were statistically signiifcant (P< 0.05 ). The ossiifcation rate of lesions was 80.9% in the ibandronate group, which was 26.5% in the control group, and there were statistically signiifcant differences between the 2 groups (P < 0.05 ).Conclusions Administration of sodium ibandronate after cyst curettage and bone grafting is a new adjuvant therapy for spinal giant cell tumor, with satisfactory clinical outcomes.
