The incidence and influencing factors of avascular necrosis of the femoral head after the closed reduction and ifxation of elderly femoral neck fractures

Abstract:Objective To study the incidence of avascular necrosis of the femoral head after the closed reduction and ifxation of elderly femoral neck fractures, and to explore the correlation between inlfuencing factors and the femoral head necrosis.Methods A total of 138 cases (≥65 years old ) of femoral neck fractures from June 2005 to December 2011 were retrospectively analyzed. All patients underwent closed reduction and internal ifxation. Complete follow-up records of 116 patients were analyzed by statistics. Gender, the type of fracture, the time from the injury to the surgery, and the quality of reduction were analyzed by multivariate Logistic regression analysis as related factors.Results Of all 116 cases, the average age was 69.5 years ( range: 65 - 78 years ) and the average follow-up time was 11.5 months ( range: 5 - 24 months ). Avascular necrosis occurred in 23 cases ( 19.8% ). The average Harris score of the hips was ( 82.3 ± 3.5 ) points ( range: 73 - 93 points ). Avascular necrosis of femoral head factor -related analysis showed: the type of fracture, the time from the injury to the surgery, the quality of fracture reduction were signiifcantly correlated to the incidence of avascular necrosis.Conclusions The femoral head avascular necrosis rate is not signiifcantly higher in the elderly after the closed reduction and ifxation, and satisfactory results can be achieved. The type of fracture, the quality of fracture reduction, and the time from the injury to the surgery are signiifcantly related to the avascular necrosis of the femoral head.
