Drainage volume of wound covered by vacuum sealing drainage after extremity salvage

Abstract:Objective To review the drainage volume of extremity salvage patient receiving VSD therapy. Methods From January 2011 to January 2015, 63 patients with major arterial injuries of the extremity ( 38 upper extremities and 25 lower extremities ) underwent artery repair, who were then treated temporarily with VSD because primary closure had not been achieved. Another 38 patients ( 16 upper extremities and 22 lower extremities ) without major arterial injury were diagnosed compartment syndrome and wounds were covered by VSD after emergency compartment fasciotomy. For each case in the two groups, wound size was measured and the daily drainage volume was recorded postoperatively for 7 days. Drainage volume per unit area ( ml / cm2 ) was compared between the two groups. Results In major arterial injury group, the average drainage per unit area ( ml / cm2 ) from day 1 to day 7 postoperatively were 1.50, 1.48, 1.35, 1.09, 0.97, 0.86, 0.94 for upper subgroup and 1.25, 0.83, 0.65, 0.74, 0.59, 0.47, 0.42 for lower subgroup. In the group of compartment syndrome, 1.21, 0.92, 0.60, 0.47, 0.42, 0.33, 0.26 for upper subgroup and 0.55, 0.58, 0.40, 0.37, 0.25, 0.24, 0.26 for lower subgroup. By independent-samples T test, the average drainage volume per unit area in the major arterial injury group were larger than those in compartment syndrome group for both upper and lower subgroup respectively. But only for the drainage of upper subgroups from day 2 to day 7 postoperatively, data showed significant differences ( P<0.05 ). And within the same group, data of upper subgroup was larger than those of the lower subgroup, but significant differences ( P<0.05 ) were only showed in major arterial injury group from day 2 to day 7 postoperatively. Conclusions For extremity salvage treated with VSD, the volume of drainage is large and needs to be strictly observed.
