Repair of soft tissue defects in the ankle and foot with peroneal perforator-based sural neurofasciocutaneous island lfap harvested by minimally invasive dissection

Abstract:Objectives To report the technique of minimally invasive dissection of the soft tissue defects repair in the ankle and foot with peroneal perforator-based sural neurofasciocutaneous lfap and the results of its clinical application.Methods From October 2011 to August 2014, 12 patients with sole defects in 8 and foot defects in 4 were treated by peroneal perforator-based sural neurofasciocutaneous lfaps which were harvested by Plasma-mediated bipolar radiofrequency ablation and turned distally at a point 1-3 cm above the external malleolus to cover the recipient wounds. The area of the skin defects ranged 5 cm×4 cm-10 cm×9 cm, while the lfap 6 cm×6 cm-12 cm×11 cm.Results All the lfaps in this series survived completely except one which presented with a partial necrosis in the distal 1 cm of the lfap. Donor wounds were covered with skin graft in 8 cases and closed by suture following incision distention in 4. Follow-up ranging from 6 months to 3 years revealed good conifguration of the lfaps and satisfactory therapeutic results.Conclusions Minimally invasive dissection of the peroneal perforator-based sural neurofasciocutaneous lfap can preserve as many as possible donor tissues minimizing the damage to the donor sites, and the lfaps can be used to repair the tissue defects in the ankle and foot with satisfactory outcomes.
