Advances in Clinical Diagnosis and Treatment of Spontaneous Spinal Epidural Hematoma

Abstract:Spontaneous spinal epidural hematoma(SSEH)is a rare and rapidly progressive spinal disorder,usually caused by rupture of the intraspinal venous plexus.It presents with severe,unexplained back pain and acute neurological deficits that can quickly evolve into paraplegia.The thoracic spine is the most frequently involved segment,and the hematoma may span multiple vertebral levels.Early diagnosis and prompt surgical intervention are critical for improving prognosis;MRI is the imaging modality of choice.Differential diagnosis must exclude epidural tumors,spinal subdural hematoma,spinal infections,and other entities.Surgery is the first-line treatment,and its efficacy is closely related to the degree of neurological impairment and the timing of intervention.
