Analysis of intraoperative complications and key technical points in inter-tan internal fixation for elderly intertrochanteric femoral fractures

Abstract:Objective To observe the surgery-related complications of Inter-Tan fixation in elderly patients with intertrochanteric fractures,and to propose surgical keypoints and corresponding strategy.Methods A retrospective analysis was carried out on 429 elderly patients with intertrochanteric fractures in Xingyi People's Hospital from March 2015 to March 2024.All patients were treated with closed manual reduction and minimally invasive insertion of Inter-tan fixation.Results Fifteen patients were suffered from internal fixation-related complications with the incidence of 2.27%,including 1 case of crossed head-neck screws and coupling failure,3 cases of head-neck screws cutting into the joint cavity,7 cases of difficult positioning of distal locking screws,and 4 cases of distal locking screws sinking into the bone.Conclusion During the surgical procedure of Inter-Tan nailing for intertrochanteric fractures in elderly patients,factors such as osteoporosis,complex fracture patterns,and different surgical techniques,may lead to complications,such as head-neck screws intercrossing,cutting into the joint cavity,or difficult positioning or sinking into the bone of distal locking screws,which requiring special attention in clinical practice.
