Clinical efficacy of elastic intramedullary nailing fixation combined with platelet-rich plasma injection in the treatment of midshaft clavicle fractures

Abstract:Objective To explore the clinical efficacy of elastic intramedullary nailing fixation combined with platelet-rich plasma(PRP)injection in the treatment of midshaft clavicle fractures,and to provide references for optimizing the treatment plan of such fractures.Methods A total of 80 patients with midshaft clavicle fractures admitted to Zhoukou Orthopedic Hospital from January 2023 to May 2024 were involved and divided into the experimental group and the control group by random number table method,with 40 cases in each group.The control group was treated with simple elastic intramedullary nailing fixation,while the experimental group was treated with additional PRP injection.The Constant-Murley shoulder score(CMS)and visual analogue scale(VAS)score for pain were compared between the two groups after surgery.Meanwhile,the fracture healing-related indicators(fracture healing time,postoperative callus score,bone mineral density)and the total incidence of complications were compared between the two groups.Results CMS of both groups gradually increased at 1,3,and 6 months after the operation,and CMS of the experimental group at each time point were significantly higher than those of the control group at the same period(P<0.05).The VAS scores gradually decreased over time(P<0.05);There was no significant difference in the VAS score between the two groups before the operation and 6 hours after the operation(P>0.05),while VAS scores of the experimental group at 1 day,3 days,1 week,and 2 weeks after the operation were significantly lower than those of the control group at the same period(P<0.05).The fracture healing time of the experimental group was significantly shorter than that of the control group,and the postoperative callus score and bone mineral density of the experimental group were significantly higher than those of the control group.The total incidence of complications in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion Elastic intramedullary nailing fixation combined with PRP injection in the treatment of midshaft clavicle fractures can effectively reduce the degree of postoperative pain in patients,accelerate the process of fracture healing,improve the recovery of shoulder joint function,and reduce the risk of postoperative complications.
