Evaluation of the effect of full endoscopic decompression and fusion with unilateral internal fixation for the treatment of lumbar spinal stenosis

Abstract:Objective To observe the clinical effect of lumbar spinal stenosis(LSS)treated with unilateral internal fixation under full endoscopic decompression and fusion of the spine.Methods A retrospective analysis was conducted on the clinical data of 80 LSS patients admitted to Puyang Oilfield General Hospital from February 2020 to February 2023.The patients were divided into two groups based on surgical methods,with 41 cases undergoing full endoscopic lumbar discectomy and fusion with unilateral internal fixation included in the study group and 39 cases undergoing traditional open surgery included in the control group.Differences in surgical indicators,visual analogue scale(VAS)score,Oswestry disability index(ODI),gait symmetry ratio,the improvement of Japanese Orthopedic Association(JOA)score,as well as incidence of complications were compared between the two groups.Results The study group had less intraoperative blood loss,shorter time to ambulation,and shorter hospital stay than the control group,while the operation duration was longer(P<0.05).After surgery,the study group had lower VAS score,ODI,higher gait symmetry ratio,and higher excellent and good rate of improvement of JOA score than the control group(P<0.05).There was no statistical difference in the incidence of complications between the two groups(P>0.05).Conclusion Compared with traditional open surgery,the treatment of LSS with full endoscopic decompression and fusion with unilateral internal fixation could alleviate postoperative pain,effectively promote the improvement of lumbar function and the restoration of gait symmetry with less trauma and faster postoperative recovery.
