Emergency repair method and efficacy observation of foot skin avulsion injury due to electric vehicle traffic trauma

Abstract:Objective To investigate the emergency repair methods and efficacy of foot skin avulsion injuries caused by electric vehicle traffic trauma.Methods A retrospective analysis was performed on 46 patients with foot skin avulsion injuries due to electric vehicle traffic accidents admitted to the 923rd Hospital of Joint Logistics Force from May 2019 to October 2023.Patients underwent two wound repair procedures after emergency debridement:anti-taken skin in-situ reimplantation with compression bandaging(control group,20 cases 20 feet)and anti-taken skin in-situ reimplantation combined with vacuum sealing drainage(VSD)(observation group,26 cases 26 feet).The operation time,dressing change frequency,wound healing,and score of visual analogue scale(VAS)for pain,American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hindfoot function score,and Vancouver scar scale(VSS)score at the last follow-up were compared between the two groups.Results The operation time of the control group was longer than that of the observation group,and the number of dressing changes was more than that of the observation group,indicating statistical differences between the two groups(P<0.05).A comparison between the two groups showed there were no significant differences in gender,age,time from injury to surgery,or follow-up duration between the two groups.The excellent rate of skin grafting in the observation group was significantly higher than that in the control group(88.46%vs 60.00%,P<0.05).At the final follow-up,no significant difference was observed in AOFAS scores between the groups,but the VSS score in the observation group was significantly lower than that in the control group[(4.85±1.35)vs(7.30±2.11)points,P<0.05],and the VAS score of the observation group was also lower than that of the control group[(1.95±1.00)vs(2.84±1.31)points,P<0.05].Conclusion For foot skin avulsion injuries caused by electric vehicle traffic trauma,anti-taken skin in-situ reimplantation combined with VSD technology can significantly improve skin graft survival rate,wound appearance,and pain relief compared with traditional anti-taken skin in-situ reimplantation with compression bandaging.
