Early efficacy of arthroscopic minimally invasive percutaneous plate osteosynthesis in the treatment of tibial plateau fractures and its influence on the oxidative stress status of the patients

Abstract:Objective To investigate the early effect of arthroscopic minimally invasive percutaneous plate osteosynthesis(MIPPO)in treatment of tibial plateau fracture(TPF)and its influence on the oxidative stress status of the patients.Methods Clinical data of 92 patients with TPF admitted to Pingmei Shenma Medical Group General Hospital from April 2021 to February 2023 were retrospectively analyzed,and they were divided into Group A(46 cases)and Group B(46 cases)according to different surgical protocols.Patients in Group A received arthroscopic MIPPO,and patients in Group B received open reduction and internal fixation(ORIF).The perioperative indexes,pre-and post-operative visual analog scale(VAS)score,Hospital for Special Surgery(HSS)knee score,gait,the levels of oxidative stress indexes[total antioxidant capacity(TAC),superoxide dismutase(SOD),reactive oxygen species(ROS)]and complications of the two groups were compared.Results Group A had less intraoperative blood loss volume and incision length,shorter ambulation time and bone healing time,and longer operation time compared to Group B(P<0.05).On the first day after surgery,the serum TAC and SOD levels of Group A were higher than those in Group B,the serum ROS level was lower than that in Group B;VAS score of Group A was lower than that of Group B at 3 days after surgery(P<0.05).HSS score,gait rate,stride frequency and stride length ratio of Group A were higher than those of Group B at 6 months after surgery(P<0.05);The incidence of complications in Group A was 2.17%(1/46),which was lower than 19.57%(9/46)in Group B(P<0.05).Conclusion Compare to ORIF for TPF patients,arthroscopic MIPPO treatment will extend the surgery time,However,it has the advantages of minimally invasive surgery with less oxidative stress damage to the body and more effective pain relief,which is more conducive to promoting the recovery of knee joint function,improving gait and reducing complications.
