Comparison of therapeutic effects between different viscosity bone cement in treating osteoporotic vertebral burst fracture

Abstract:Objective To explore the differences of clinical effects between different viscosity bone cement percutaneous kyphoplasty(PKP)for the treatment of osteoporotic vertebral burst fracture(OVBF).Methods Clinical data of 150 OVBF who performed PKP in Yongcheng People's Hospital from April 2018 to January 2023 were retrospectively analyzed.Patients were divided into high-viscosity group(n=77)and low-viscosity group(n=73)according to different viscosity bone cement treatment,in which during PKP procedure,patients in high-viscosity group were given high viscosity bone cement,while patients in low-viscosity group were given low viscosity bone cement.Operation related indexes of patients between two groups were compared,preoperative and postoperative bone mineral density(BMD),serum bone metabolism indexes[osteocalcin(OCN)and C-terminal cross linked telopeptide of type Ⅰ collagen(CTX-Ⅰ)],visual analogue scale(VAS)score,Oswestry disability index(ODI)and bone cement leakage of patients were evaluated in two groups.Results The operation duration and intraoperative fluoroscopy times in high-viscosity group were(38.97±6.55)min and(4.53±0.53)times respectively,which were both shorter than those in low-viscosity group[(49.58±7.82)min and(5.12±0.64)times],with statistical differences between two groups(P<0.05).Serum OCN and CTX-Ⅰ levels at 1,3 months postoperatively as well as VAS scores and ODI at 3,6 months postoperatively in the two groups all improved when compared with those before the surgery(P<0.05).One month after surgery,the serum OCN level in the high viscosity group was(14.93±1.20)ng/mL,which was higher than that in the low viscosity group(14.52±1.22)ng/mL,and the CTX-I level was(290.51±21.35)mg/L,which was lower than that in the low viscosity group(302.54±22.37)mg/L,with statistically significant differences(P<0.05).During the follow-up,the incidence of bone cement leakage in high-viscosity group(5.19%)was lower than that in low-viscosity group(16.44%),with a statistical difference between two groups(P<0.05).Conclusions Both high-viscosity and low-viscosity bone cement could improve vertebral function and alleviate postoperative pain for OVBF patients who underwent PKP procedure.Compared with low-viscosity bore cement,high-viscosity bone cement could reduce operation time and intraoperative fluoroscopy frequency,improve postoperative patients'early bone metabolic status,and lower the risk of bone cement leakage.
