Application of anterior auxiliary plate fixation in the treatment of unstable femoral intertrochanteric fractures

Abstract:Objective To compare the clinical results of single-plate (proximal femoral anatomic locking plate, PFLP) fixation versus double-plate (PFLP + anterior auxiliary plate) fixation in the treatment of unstable femoral intertrochanteric fractures. Methods Clinical data of 142 patients with unstable femoral intertrochanteric fractures, who underwent open reduction internal fixation with PFLP from August 2010 to August 2014 in the 175th Hospital of PLA, were retrospectively analyzed. According to different fixation methods, the patients were divided into two groups, double-plate group (PFLP + anterior auxiliary plate, 46 patients) and single-plate group (PFLP, 96 patients). The differences of clinical efficacy and postoperative complications were compared between two groups. Results All patients were followed up for 20 to 48 months [mean, (25 ± 3) months]. Operation time and intraoperative estimate blood loss in single-plate group were less than those in double-plate group, there were statistical differences between two groups (P <0.05). At 1 year after the operation, the number of cases with fixation failure, hip varus deformity or bone nonunion, fracture healing time, Harris hip score of double-plate group showed significant superiority compared with those of single-plate group (P <0.05). Conclusion For unstable femoral intertrochanteric fracture, double-plate fixation technique, although to some extent, increasing operation time and intraoperative estimate blood loss, could provide more reliable fixation, reduce the rates of implant failure and malunion, and promote bone healing and improve hip joint function.
