Therapeutic effect analysis and technological summary on unilateral percutaneous vertebroplasty for thoracolumbar osteoporotic vertebral compression fracture

Abstract:Objective To evaluate the effects of unilateral percutaneous vertebroplasty (PVP) in treating thoracolumbar osteoporotic vertebral compression fracture (OVCF), and to summarize the key points of PVP puncture and cement injection procedures. Methods Clinical data of 108 patients (154 vertebrae) of thoracolumbar OVCF treated with unilateral PVP from May 2011 to March 2013 in Zhengzhou Orthopaedics Hospital were analyzed retrospectively. The improvement of pain symptom was evaluated by visual analogue scale (VAS), and the patients' activity was measured by the scale of activity of daily life (ADL). Results All patients underwent the surgery smoothly. The mean operation time was 55 min (35-90 min); The mean cement injection volume was 3.9 mL (1.5-5.8 mL) in thoracic vertebrae and 4.7 mL (2.7-7.0 mL) in lumbar vertebrae. Cement leakage was found in 17 vertebrae (11.0%), including 9 in intervertebral space, 7 in anterior vertebral or paravertebral position, and 1 in vertebral canal (which was found by CT scanning 3 months after the surgery). No neurological symptoms had happened. The mean follow-up period was 14.2 months with the range of 13-28 months. At one year follow-up, VAS and ADL scores were improved [(1.71 ± 0.28) and (90 ± 13)] compared with the preoperative ones [(7.80 ± 1.43) and (49 ± 11)]. Conclusions Unilateral PVP is an effective and safe procedure for treating thoracolumbar OVCF which could relieve pain rapidly, improve the activity of daily life significantly;As for PVP operation techniques, puncture and cement injection should be mastered skillfully.
