Abstract:Objective To analyze the immune reconstitution status and influencing factors among HIV/AIDS patients receiving antiretroviral therapy in Jiaxing, and provide a theoretical foundation for further optimizing antiretroviral therapy efficacy. Methods A retrospective cohort study design was employed to conduct descriptive analysis of epidemiological characteristics among 1 622 HIV/AIDS patients.Cox proportional-hazards regression models were utilized to analyze influencing factors of immune reconstitution following antiretroviral therapy. Results Following antiretroviral therapy, immune reconstitution was achieved in 35.8%of patients.Comparative analysis revealed that patients with BMI>25.0-30.0 kg/m2,baseline CD4 T-cell counts>200 cells/μL,baseline viral loads 0-1 000 copies/mL,treatment durations>1 year, and age at diagnosis 0-45 years exhibited significantly higher immune reconstitution rates compared to those with BMI>18.5-25.0 kg/m2,baseline CD4 T-cell counts 0-200 cells/μL,baseline viral loads>1000 copies/mL,treatment durations 0-1 year, and age at diagnosis>45 years. Conclusion Multivariate analysis identified BMI,baseline CD4 T-cell count, baseline viral load, treatment duration, and age at diagnosis are influencing factors of immune reconstitution in HIV/AIDS patients. It is recommended to emphasize early antiretroviral therapy initiation, longitudinal adherence management, and systematic CD4 monitoring protocols, with particular vigilance for patients exhibiting low BMI or advanced age at diagnosis.
