Research on the impact of electronic health record filing and utilization on the management efficacy of chronic diseases

Abstract:Objective Taking hypertension and type 2 diabetes as examples,to explore whether the use of electronic health record(EHR)filing can improve the effectiveness of chronic disease management,aiming to provide references and suggestions for the development and improvement of the integration of medical and preventive services.Methods The basic public health service index data of 12 districts and counties in Sanming from 2018 to 2023 were utilized.Generalized estimating equations and Sobel tests were employed to investigate the impact mechanism of EHR filing and utilization on the management effectiveness of chronic disease patients.Results The EHR utilization rate played a complete mediating role between the EHR filing rate and the standardized management rate of hypertension and type 2 diabetes patients(β=0.271;0.267,P=0.001),and a partial mediating role between the EHR filing rate and the blood pressure/blood glucose control rate of the managed population(β=0.223;0.271,P<0.01).The comprehensive reform of the disease control system positively moderated the relationship between the EHR utilization rate and the standardized management rate of hypertension,type 2 diabetes patients(β=0.067;0.081,P<0.001).Conclusion The filing and utilization of EHR is a fundamental requirement and a driving tool for enhancing the capacity of chronic disease health management.It is necessary to continuously promote the standardization of EHR and open them to residents,while innovating integration between medicine and prevention through the comprehensive reform of the disease control system.
