Abstract:In order to explore the practical operation of exercise referral schemes(ERS)in the primary health care system, based on domestic and foreign research, practical experience and on-site interviews,it was found that the integration of physical and health services has not yet been included in the evaluation of community-level medical and health care institutions, there is a lack of basis for funding, complex administrative affiliation, unclear division of labor, weak cross departmental collaboration, and lagging standard construction and quality monitoring.Based on the establishment of a continuum of physical and health integration,it aims to discuss the internal operating mechanism and external linkage system of ERS,determine the roles and responsibilities of ERS coordinators, and determine the qualifications of relevant practitioners, incorporate physical activity levels into vital signs, use sports prescriptions as"drugs".The process of sports intervention follows traditional clinical treatment pathways for sports intervention, from an operational perspective, proposes a suitable process for ERS operation in the current economic and social development context, solves the problem of actual execution of exercise prescriptions, highlights the importance of supporting standards, payment, insurance, supervision, and other security elements for ERS,and provides environmental support for incorporating ERS into the primary health care system.
