Abstract：Chronic diseases are the main cause of threatening the health of residents.The management measures for patients with chronic diseases based on the intervention mapping can improve the self-management ability of patients with chronic diseases and improve the quality of life.The review summarizes article.the concept,implementation process and application effect of intervention mapping in the management of patients with chronic diseases,in order to provide guidance for improving the management of patients with chronic diseases in China.
