Clinical Characteristics and Analysis of Hospitalized Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease During Influenza Season

Abstract:OBJECTIVE To explore the clinical characteristics of hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)during influenza seasons and provide reference for diagnosis and treatment of influenza virus in patients with acute exacerbation during influenza seasons.METHODS A retrospective analysis was performed on 3 985 patients hospitalized for acute exacerbation of COPD from January 2015 to December 2021.Weekly influenza intensity data ILI%(influenza-like illness)was obtained from the National Influenza Surveillance Center website.Patients admitted during weeks with ILI%≥4.1%were assigned to the influenza season group(influenza group,n=758),and those admitted during weeks with ILI%<4.1%were assigned to the non-influenza season group(non-influenza group,n=3 227).Binary logistic regression was used to analyze risk factors for patient mortality.RESULTS Mean peripheral blood eosinophils(EOS)in the influenza group was(0.09±0.13)×109/L versus(0.10±0.17)×109/L in the non-influenza group,with statistically significant difference(t=2.792,P<0.05).Mean length of stay in the influenza group was(10.33±6.32)days versus(9.28±5.52)days in the non-influenza group,with statistically significant difference(t=4.555,P<0.01).Hospitalization costs were(10 068.52±8 163.41)yuan in the influenza group versus(8 634.08±5 975.90)yuan in the non-influenza group,with statistically significant difference(t=5.511,P<0.01).Mortality rate was 4.84%in the influenza group versus 2.67%in the non-influenza group,with statistically significant difference(t=8.597,P<0.01).Logistic regression analysis showed that influenza season onset,advanced age,cardiac comorbidities,and smoking were independent risk factors for mortality.CONCLUSION Influenza virus infection is an important factor in the increase of acute COPD exacerbations during influenza season.Influenza virus-induced acute exacerbations are characterized by decreased EOS,prolonged hospital stays,increased hospitalization costs,and higher mortality rates.Attention should be paid to the diagnosis and treatment of influenza virus-induced acute exacerbation of COPD.
