Full-Process Management Model for Cognitive Impairment Based on the Integrated Care for Older People Concept

Abstract:The Yuyuan Community Health Service Center,guided by the Integrated Care for Older People(ICOPE)concept,has developed a full-process service model that integrates prevention and treatment of Mild Cognitive Impairment(MCI).The model emphasizes people-centered integrated services and builds a humanized care system through coordination and cooperation.The Yuyuan Street Community Health Service Center in Huangpu District established the first community-based cognitive impairment clinic in Shanghai and collaborates with multiple medical institutions to provide comprehensive intervention management.The innovative system has covered aspects from prevention,screening,diagnosis,treatment,rehabilitation and care,integrating various resources to form a collaborative network for cognitive impairment prevention and treatment.The memory clinic plays a crucial role in cognitive impairment prevention and treatment,offering comprehensive and continuous services to patients,and provide valuable experience for exploring a community-based dementia management approach with Chinese characteristics.
