Abstract：In view of the current situation of primary medical and health institutions in Shanghai facing the construction requirements of"community health management center","community rehabilitation center"and"community care center",but with relatively limited institutional resources,the study considered how to maximize the use of existing resources through reasonable allocation and accurate layout,reduce trial and error,and quickly clarify the construction path of the three major community centers.Finally,through combing the functional positioning of the three major community centers and the actual working scenarios of primary medical and health institutions,the positions of"community health management center","community rehabilitation center"and"community care center"in the development of institutions,the order of business service output of the three,and the center-site double-layer service mode were clarified.Under the mode,the two service output levels of primary medical and health institutions each clearly defined their roles,and worked together to create a center research and development-site service construction mode.
