Influencing Factors Associated with Health-care Seeking Delay and Diagnosis Delay of Pulmonary Tuberculosis Patients in Sanyuan District of Sanming from 2017 to 2023

Abstract:OBJECTIVE To understand the current status of health-care seeking delay and diagnosis delay in patients with pul-monary tuberculosis(TB)in Sanyuan District of Sanming from 2017 to 2023,analyze the underlying factors associated with these de-lays,and to provide a scientific basis for tuberculosis prevention and control.METHODS The data of pulmonary tuberculosis patients in Sanyuan District of Sanming from 2017 to 2023 was collected through the China Tuberculosis Information Management System to describe the distribution and change trend of the delay in health-care seeking and diagnosis.Using χ2 test,and multivariate logistic regression model analyzed the influencing factors of the delay in health-care seeking and diagnosis.RESULTS From 2017 to 2023,there were 607 cases of pulmonary tuberculosis in Sanyuan District registered.The median health-care seeking time of TB patients in Sanyuan District of Sanming city was 10(1,36)d,and the health-care seeking delay rate was 42.50%.There was no statistically significant trend in the delay of health-care seeking.The median diagnosis time was 20(9,36)d,and the diagnosis delay rate was 63.10%,with an upward trend.Compared with local,non local registered residence was associated with an increased risk for health-care seeking delay(OR=1.877,95%CI:1.250~2.818,P<0.05).Compared with the age group<25 years old,the age group≥60 years old has an increased risk of delayed diagnosis(OR=2.298,95%CI:1.222~4.323,P<0.05).Compared with ac-tive visits,there is an increased risk of delayed diagnosis in referral(OR=2.153,95%CI:1.124~4.126,P<0.05)and follow-up(OR=5.515,95%CI:1.554~19.570,P<0.05).Compared with those who did not use molecular biology tests,those who used mo-lecular biology tests had an increased risk of delayed diagnosis(OR=2.577,95%CI:1.671~3.974,P<0.05).CONCLUSION The delay in health-care seeking and diagnosis of pulmonary tuberculosis in Sanyuan District is still common.The next step should be to strengthen the health education of the transient population and tuberculosis inspection of the elderly,to reduce the occurrence of de-lay in health-care seeking and diagnosis of pulmonary tuberculosis.
