A Study of Urban-rural Differences in the Utilization of Community-based Elderly Services for The Disabled Elderly and Their Influencing Factors

Abstract:OBJECTIVE Based on the urban-rural comparative perspective,to explore the utilization of community-based elderly services for the disabled elderly in urban and rural areas and their influencing factors.METHODS Using data from the 2018 China Health and Retirement Longitudinal Study,4 394 eligible elderly people with disabilities were included in the study,and the χ2 test was used for one-way analysis and logistic multiple regression analysis to target the factors affecting the utilization of community-based elderly services by the elderly people with disabilities,and the impact of the utilization of community-based elderly services was compared between urban and rural areas through heterogeneity analysis.RESULTS The utilization of community elderly care services for the disabled elderly was low in both urban and rural areas in China,and the utilization of community elderly care services for the disabled elderly in urban areas was slightly higher than that in rural areas,and the regression results showed that the education level,the number of caregivers,the pension,the status of signing a contract with a family doctor,the number of chronic diseases,the region,and the level of depression scores had an impact on the utilization of community elderly care services.Significant factors in the urban and rural samples differed,and family doctor contracting status and region were common influences in urban and rural areas and had a greater impact in rural areas.CONCLUSION In order to improve the utilization rate of community nursing services,it should strengthen the financial support and resource supply for the construction of the disabled function and tilt it in the central and rural areas,increase the popularization of the publicity in the rural areas,improve the economic accessibility of the service utilization in the rural areas,explore the model of combining the community health care services with the management of chronic diseases as the core,and expand the diversification of mental health service.
