A Study on Pathogen Detection in Newly Treated Pulmonary Tuberculosis Patients in Shijingshan District of Beijing

Abstract:OBJECTIVE To analyze the results of sputum samples of newly diagnosed tuberculosis cases in Shijingshan District Institute of Tuberculosis Prevention from 2022 to 2023,and to explore the factors affecting the etiologically positive rate of tuberculosis.METHODS The data of 151 suspected pulmonary tuberculosis cases from 2022 to 2023 were collected,including 60 pulmonary tuberculosis cases and 34 etiologically positive cases.Using clinical diagnosis as the standard,compare and analyze the results of sputum smear acid fast staining microscopy,improved Roche solid culture method,and GeneXpert detection,and evaluate the effectiveness of the three detection methods.RESULTS The positive detection rates of sputum smear,culture method,and GeneXpert were 21.67%,30.00%,and 51.67%,respectively.Compare the positive detection rates of GeneXpert with sputum smear and culture method using chi square test,and the differences are statistically significant(χ2=104.072,4.117,P<0.05).The sensitivity,conformity rate,and negative predictive value of GeneXpert were 51.67%,79.47%,and 75.42%,respectively,which were superior to sputum smear and culture methods.The consistency test between sputum smear,culture method,GeneXpert and clinical diagnosis was conducted,and the Kappa values were 0.25,0.361,and 0.563,respectively.GeneXpert was moderately consistent with clinical diagnosis results and superior to sputum smear and culture method.CONCLUSION The application of GeneXpert has significantly improved the detection rate of Mycobacterium tuberculosis and the diagnostic efficiency of rifampicin resistance in Shijingshan District.Measures still need to be taken to improve the positive rate of pulmonary tuberculosis etiology in this area.
