Investigation on Self-management of Hypertension and Diabetes in Community

Abstract:OBJECTIVE To study the self-management of hypertension patients with diabetes in Zhangyan Town of Jinshan Dis-trict of Shanghai,and provide evidence for community family doctors to intervene.METHODS 457 patients(191 males,266 fe-males)with hypertension and diabetes who were managed by Zhangyan Town in the new chronic disease integration system of Shanghai were randomly selected to carry out a questionnaire survey.RESULTS The total scores of patients with different education levels were higher than those of patients with diet,exercise,medication and self-management(P<0.05).There were statistically significant differencesamong patients of different ages in terms of whether they were clear about their daily medication time,wheth-er they could identify fake and inferior drugs,and whether they understood the physiological characteristics and medication specific-ity of the elderly(P<0.05).CONCLUSION The self-management project of hypertension patients with diabetes at different ages is not ideal.It is suggested to carry out scientific,accurate,standardized and effective health management through the health self-management team and the family doctor team,enhanced the health awareness of patients,promoted their good living and eat-ing habits,achieved effective control of blood sugar and blood pressure,and reduced complications.
